Black Hills Surgery Center is pleased to announce a program that will provide individuals under all health plans the ability to access services of Black Hills Surgery Center at no added cost. As you may know, some health plans use economic pressure to encourage patients to go to certain healthcare providers. Black Hills Surgery Center may not be included in the provider panel of all health plans. This might mean when patients choose to come to the Black Hills Surgery Center for non-emergency healthcare, they may pay a greater portion of their bill in the form of a higher co-insurance and/or deductible.

Black Hills Surgery Center will waive the non-participating provider penalty for these individuals. Patients will be responsible for paying us only the usual participating provider deductible and co-insurance as provided in their benefit plan. Black Hills Surgery Center wants to preserve patients' freedom to choose where they wish to obtain their healthcare without incurring a higher out-of-pocket cost.

Here are the details:

• To be eligible for this special arrangement, patients must not have any Medicare coverage – secondary or primary.

• At the time of registration, patients will be responsible for presenting their insurance card and agreeing, in writing, to pay the standard deductible and co-insurance in their plan, exclusive of any non-participating provider penalties.

• The patient will agree, in writing, to assign to Black Hills Surgery Center the right to collect the payment owed by the provider for the services the patient receives and, if payment is nonetheless made to the patient, to pass along that payment to Black Hills Surgery Center together with the accompanying Explanation of Benefits from the provider.

• Neither the patient, the employer, nor the provider will be required to pay the non-participating provider penalty.

While this special arrangement requires Black Hills Surgery Center to absorb additional expenses, we view this as an investment in our community's health. Black Hills Surgery Center believes freedom of choice is critically important to the residents of this community.

Sincerely,

William A. May
